How Booking Your First Appointment Works
We make it easy for new patients to schedule their first appointments and begin their journey toward improved skin. Here’s how it works:
Activate your portal once you receive email after booking appointment.
For new patients, please bring completed paperwork and arrive 20 minutes prior to your appointment.
Bring proof of insurance and a driver's license.
If the patient is a minor, their parent or guardian must sign a release before the appointment.
If you need to cancel or reschedule, we kindly ask you to call the office 24 hours prior to your appointment or be subject to a cancelation fee.
We collect co-pays, deductibles, and past balances prior to your appointment. We accept cash, checks, Visa and Mastercard. We do not accept Care Credit or American Express at this time.
Due to being a small medical business, We are now adding a Credit Card on file to be seen. Your Credit Card will be encrypted and no one will be able to see your information. Your Credit Card will NOT be charged without receiving a statement and will never be charged over what your allowable is with your insurance.

Dr. Kuykendall also has an excellent team who is well trained at scheduling your appointment as quickly as possible as we all understand how worried patients can be with lesions of concern or miserable with rashes. Our front desk is excellent at trying to get you in same day if needed in any cancellation slots.
At your appointment we always ask why you are being seen and try to focus on your main concern at your appointment while also offering skin checks to everyone. We always do our best to biopsy as many lesions of concern at one appointment without having to schedule multiple appointments. We are here to provide different treatment options and discuss risks versus benefits of each.
